Let's try to answer the question first: Who is a proctologist? And let's explain a little about the basic concepts.

Proctology

Proctology is a medical specialty that deals with the diagnosis and treatment of diseases of the rectum, anus and lower colon.

Who is a proctologist - meaning

Proctologists, doctors specializing in this field, treat a variety of conditions such as hemorrhoids, anal fissures, abscesses, fistulas, polyps, and rectal cancer. The goal of proctology is to provide comprehensive care for patients with these health problems.

Proctologist is in fact an archaic name, in modern surgery the word proctologist rather falls under the title of colorectal surgeon. And here the question immediately comes up: How can an outpatient physician be a colorectal surgeon? Well it can't, it's not technically possible. In Slovakia there is no specialized training in the field - colorectal surgery or if you want to be more archaic - coloproctology.

Proctologist Specialist

A proctologist should first and foremost be an expert in his field with many years of experience. Secondly, he should have a humane approach, moral principles, and you will certainly take into account the environment you are coming to. "After all, I'm going to a private practice."

What is proctology and how is it studied?

Let's go at it from the beginning. You graduate from medical school (it's a memorable 6 years) and get your MD. This is the key moment when you have to, or should, decide what's next for you and what kind of professional practice you will do. If you choose surgery, you will have another 5 years to prepare for the specialty exam. You have to count on several years of experience in a surgical department, but that does not start with colorectal, advanced surgery, but rather with administrative work, from admitting patients to discharging them, including countless assisting senior surgeons, including the aforementioned colorectal surgeons. Most young surgeons at this time don't think about whether they will be a colorectal surgeon someday. They need to learn to operate on basic surgical diagnoses, whether elective or acute. After the 5th year, if you meet all the requirements and have performed a certain number of basic surgical procedures, you will be admitted to the specialty examination in general surgery. You take a specialised course, which consists of a defence of your specialisation thesis and a specialised examination. If you pass the exam as a "reward" you will receive a stamp with the doctor's code in the specialty of surgery. Do you think you are already a colorectal surgeon - proctologist? Well you are not, and you can't be, because you have no experience in the rather complex subject of colorectal surgery. And now what?

"I've got a stamp, I can start a private practice can't I? Proctology or colorectal surgery as a certification activity does not exist in Slovakia, so why shouldn't I call myself a proctologistAfter all, I'll work some more time in surgery in a smaller town where they don't even do colorectal surgery and start a private practice or actually I probably don't even want to do surgery and I'll do another job temporarily. Then I'll change my mind and go back into surgical practice. In translation, I have no practice again even just in basic surgery. I'm going to set up a surgical practice and call myself a proctologist - a colorectal surgeon."

Do you think this is a real proctologist? Well certainly not, it's not technically or practically possible.

Coloproctology

But let's go back further to the story of a colorectal surgeon who actually performs colorectal surgery and proctology in his practice and has a track record of results. That's what you're looking for, isn't it?

So, we did the specialty exam in surgery and we want to be a colorectal surgeon or a proctologist. What's next? Are we at least halfway there? We're not! It's just starting, and it's starting to be a colorectal surgeon at all, you need support from the chief surgeon and from senior colleagues, colorectal surgeonsif there is one in the workplace at all and understands proctological issues. In addition, he or she must be willing to sell you some of that experience.

Can anyone do colorectal surgery or proctology? No, I don't think so. We all have some aptitudes and skills, we all excel at something different and just because you choose to do surgery doesn't mean that your skills will make you a good surgeon in this case a colorectal surgeon. But in the time leading up to the specialty exam and in the period after the specialty exam, the department head and the surrounding area sees if you have the personality for colorectal surgery.

Coloproctologist - doctor and practice

Okay, so let's say you've got support from the neighborhood and you get the opportunity to practice colorectal surgery. The journey is just beginning, with assistantships and gradually gaining experience from basic colorectal surgery to difficult procedures in the small pelvis. The prerequisite, in addition to mastering surgical techniques, is patient care in the perioperative period and in the outpatient setting. The contact with the patient from the complex diagnosis with the mastery of the rather difficult issue of onco-surgery to the dispensary care is extremely important. Gradual improvement in surgical technique in order to minimize possible postoperative complications and maximize the oncological aspect of treatment. Managing, preventing and resolving complications that are often fatal in colorectal surgery. After years of experience in the above, after a number of operated patients from difficult abdominal procedures to technically demanding procedures in the anal canal area, we can say that you are a colorectal surgeon - proctologist. And when will you actually become one? When a senior colleague you've learned from comes to you and asks for your opinion or asks for help with a complication. Colorectal surgery - proctology includes mainly challenging procedures in the small pelvis and perianal area, including laparoscopic procedures. I certainly don't mean hemorrhoids and anal fissures. Treating hemorrhoids and anal fissures are the most necessary procedures for an advanced colorectal surgeon.

How to find the right proctologist?

I think that's the kind of advanced proctologist you're looking for. Not a self-proclaimed proctologist who last saw an open abdominal cavity or complex perianal fistula just after a specialty exam and never operated on it. A colorectal surgeon - proctologist, who deals with difficult procedures in his daily practice, will never have a problem to deal with banal proctological procedures, taking into account the minimization of complications and refinement of technique.

I suppose that's exactly the kind of proctologist you need. In my opinion, a proctologist cannot be a surgeon who sits in an outpatient clinic 5 days a week. It's not technically possible. He can do basal proctology procedures, but with all due respect a proctologist, a colorectal surgeon certainly is not. Therefore, if you are choosing a doctor for your problem, the above should definitely be taken into consideration.

Proctology - colorectal surgery is a demanding field that requires years of study, dedication and, most importantly, practice. He will go through a lot of challenging situations, resolve a number of often serious complications that will leave their mark on him.

What is a proctological examination?

A proctological examination is a medical examination aimed at diagnosing and treating diseases of the rectum, anus and lower part of the colon. This type of examination is performed by a specialist called a proctologist. The most common problems that require a proctological examination include hemorrhoids, anal fissures, polyps, abscesses, fistulas, and rectal cancer. The examination may include a physical examination, digital rectal examination (DRE), anoscopy or sigmoidoscopy.

How is the proctological examination performed?

The proctological examination begins with a consultation where the doctor obtains a detailed history of the patient, including symptoms and their duration, medical history and any previous treatments. The consultation is followed by a physical examination in which the patient assumes a position that allows access to the rectum. The most commonly used position is on the side with the knees bent or the so-called gynaecological supine position.

What does a proctologist do?

The proctologist performs a digital rectal examination (DRE), in which a gloved finger is used to gently palpate the rectum and lower part of the colon to detect any abnormalities. If a more detailed examination is needed, an anoscope, a short lighted tube that allows a better view of the inside of the rectum, is used. If necessary, this may be followed by a sigmoidoscopy, which allows the lower part of the colon to be examined. After the examination, the doctor will review the findings, make a diagnosis and suggest an appropriate treatment plan.
